The Charitable Contribution Request Form, either the online or printed version, should be used for all requests for support of organizations and/or upcoming events. Your request must fall within the parameters of one of the programs listed below. Please allow 30 days for a decision and processing check request.

Our foundation bases the importance of consideration for its Charitable Donations on programs that:

  • Promote affordable housing
  • Provide economic revitalization
  • Promote the health for vibrant neighborhoods
  • Provide caring, quality and culturally appropriate health services
  • Provide shelter and emergency services to homeless families
  • Promote financial education
  • Enhance opportunities for culturally diverse people
  • Enhance quality of life through educational, social, community and economic development efforts
